Is Passive Vaping An Issue?

vaping-vamps-8Dr. Farsalinos, a leading researcher into the effect of ecigarettes, recently wrote about the latest study involving passive vaping and the nicotine levels given off by ecigarettes.

Here it is:

A new study evaluating passive vaping has recently been published in the journal Environmental Research. The study evaluated nicotine levels in the house of vapers and smokers (compared to non-smokers), and measured salivary and urinary cotinine levels in non-smokers who were exposed to tobacco and electronic cigarette use at their homes. The main finding of the study was that “passive vaping” results in nicotine absorption from non-smokers non-vapers, at similar levels as those exposed to smoke from tobacco cigarettes. Those exposed to more than 7 tobacco cigarettes per day had higher cotinine levels (thus, more nicotine was absorbed through passive exposure).

First of all, there is no surprise that nicotine is released to the environment. Since there is a lot of vapor exhaled, and considering that nicotine absorption is lower compared to smoking, nicotine is probably exhaled by the user (I say probably, because we need more evidence to be certain about that). However, we should assess the health implications of exposure to nicotine at such levels.

Does it mean that passive vaping may lead to nicotine dependence?

Does it mean that nicotine is absorbed to such levels that it may cause harm to bystanders?

The answer to both questions is NO. Passive exposure to electronic cigarette resulted in median salivary cotinine levels of 0.24ng/ml, while in the control group (no exposure to tobacco or electronic cigarette) it was 0.05ng/ml. In smokers, levels of salivary cotinine exceed 300ng/ml, especially in  smokers of >20 cigarettes per day. Therefore, the level of cotinine in “passive vapers” is approximately 1200 times lower than active smokers. The same research group measured cotinine levels in smokers few years ago, finding 146ng/ml in smokers of 15 cigarettes per day. This is 610 times higher than the levels in “passive vapers”. Since cotinine is directly associated with the total amount of daily nicotine intake, and assuming that smokers of 15 cigarettes per day get 15mg of nicotine and show 146ng/ml cotinine levels, we can calculate that passive vaping leads to daily nicotine intake of 0.025mg. Such a levels is not only harmless but has absolutely no biological effect, even according to the strictest regulatory definitions.

The European Food Safety Authority (EFSA) has defined the Lowest Observed Adverse Effect Level (LOAEL) of nicotine. This limit has a TOXICOLOGICAL ENDPOINT OF HEART RATE ACCELERATION, which is wrong because heart rate acceleration does not imply any long-term adverse effect. According to the definition, NOAEL (which is a much lower level compared to LOAEL) is defined as: “An exposure level at which there are no statistically or biologically significant increases in the frequency or severity of adverse effects between the exposed population and its appropriate control. Some effects may be produced at this level, but they are not considered as adverse, nor precursors to adverse effects. Thus, the definition by EFSA it is not in reality a LOAEL (or even a NOAEL), but much lower than that. The level set by EFSA was 0.008mg/kg body weight for ingestion, derived from calculations of intravenous nicotine injections, which found that administering 0.0035mg/kg body weight produced an acute acceleration in heart rate. For an average 75kg human, that is 0.26mg (10 times higher than the calculated 0.025mg/day intake from passive e-cigarette exposure).

In conclusion, the levels of nicotine absorbed from “passive vaping” are not only harmless but do not even produce any biological effect (not even heart rate acceleration). Considering the possibility that allowing e-cigarette use in public places may motivate smokers to switch to e-cigarette use, there is no scientific basis for any bans on e-cigarette use in public places.

American Heart Assoc. Says Vaping Is Safer Than Smoking

.0258The American Heart Association isn’t known for being all that into smoking. But vaping? It can get down with it, in certain cases.

 

The group has taken a remarkably measured stance on e-cigarettes, suggesting in a lengthy statement this weekend that the technology could help smokers quit, and refraining from outright condemning the tech, like other organizations have.

 

It’s something of a win for the industry, which is expected to top $5 billion in revenues this year. At every turn, health groups and politicians have taken shots at vaping and the companies who make e-liquid, suggesting that they are every bit as dangerous as cigarettes, despite there being some evidence (and the common sense-argument) that they’re leaps and bounds safer than combustibles.

 

The AHA had avoided making any sort of statement until now. In a 20-page policy paper, the association cites research that suggests vaping is less dangerous than smoking and suggests it can be used as a smoking cessation aid.

 

“E-cigarettes either do not contain or have lower levels of several tobacco-derived harmful and potentially harmful constituents compared with cigarettes and smokeless tobacco,” it states. E-cigarettes also “present an opportunity for harm reduction if smokers use them as substitutes for cigarettes.”

 

That’s not to say that the AHA wholeheartedly endorses the use of e-cigarettes. Like many other health organizations (and like some in the industry itself), the group suggests that e-cigs should be regulated much like tobacco products are now, and it also cites the oft-stated worry that e-cigs could “renormalize” tobacco use and serve as a gateway for children and teens to get into smoking.

Reynolds sponsors study of e-cig vapor smoke

1. I am experiencing a short cartridge life. Why is that and how can I fix it?

Normally, one cartridge lasts for about 150-200 puffs. Following are some tips that could help you achieve a longer cartridge life.

Don’t leave cartridges open after removing them from their sealed package or assembled e-cigarette. If you are not using the cartridge, make sure to put rubber caps on the cartridges.
Avoid extreme temperatures. Temperatures <60F and >80F could result in the liquid evaporating and shorter cartridge life.
If you travel frequently with your e-cigarette, it is important to use a portable carry case. Transporting the assembled e-cigarette without a case can cause it to be engaged unintentionally with airflow being moved through the unit, such as in a shirt or pants pocket.
If you don’t plan to use the unit for more than a day, unscrew the cartridge from the battery and seal with the rubber cap. Then store both the battery and cartridge inside a portable case.

  • Number of puffs from each eCig cartridge varies based on individual smoking habits. A regular cigarette has about 10-12 puffs. Depending on your smoking style, shorter or greater life could be experienced than expected from a e-cigarette cartridge.
  • Cartridges should be used within 5 days after opening.
  • Cartridge shelf life is 12 – 18 months from their production date. Our cartridges are normally less than 2 months old when we ship them.
  • After opening, store the cartridges in or close to horizontal direction with rubber-caps on.

2. Why does my cartridge heat up?

Electronic cigarettes are more like electric cigarettes: they operate on a simple yet fairly strong electric current to generate vapor. The reason for this is two-fold:

E-cig companies aiming for more vapor: In an effort to increase vapor production electric cigarettes also emit more heat than ever before. The problem can be improved by using better heat insulation on the body of the cartridge but most manufacturers using metal casing.

Heavy smokers: Most smokers – especially those who try ecigs for the first time – tend to draw longer, harder puffs. The same heat that generates vapor also heats up the cartridge. This is the main reason why ecig smokers face this warming up of the cartridge. This problem is made worse by users trying to draw too many puffs in rapid succession. The heat is often worse when the cartridge does not have much liquid left.

Manufacturers are still a little behind as they try to balance the heating problem with the demand for vapor. A majority of the vaping customers do not have a major problem though with heating and vapor production is considered as being more desirable.

3. What should I do if some of my cartridges are leaking?

The first thing we strongly recommend is to immediately throw away those cartridges. We have had very few cases of leakage in our cartridges but if that happens, we recommend that you first immediately wash your mouth many times with water.

One other thing we would recommend. After you have opened a cartridge pack, we recommend that you store these carts on a horizontal (flat surface). Don’t keep these cartridges vertically – this could lead to a liquid coming down due to gravity (if by chance there is a little extra liquid in the cart). Also, we recommend that when smoking, keep the cigarette horizontal or close to horizontal. Smoking vertically sometimes may result in possible leakage. Again it should not happen but it is just a recommendation to avoid any potential issues like this.

4. What should I do if my cartridges don’t produce enough vapor?

Sometimes the brand new cartridges have a blocked air passage. We suggest to our customers to try the following:
With the cartridge screwed on to the battery, keep the LED side of the battery in your mouth (about half to one inch) and then blow hard onto it a couple of times. You should see smoke coming out of the other side of the cartridge. Now just try inhaling your cigarette normally and you should see the vapor coming out. Please try this a couple of times and if it still does not create vapor, please contact us so that we could send you an appropriate number of replacement packs.

5. How do I clean my electronic cigarette battery?

 
It is important to clean and maintain your electronic cigarette to ensure maximum performance and a longer life. The battery should be cleaned periodically depending on how often you use it. We recommend to clean it once a week.

Below is the method that we recommend:
1. Inspect the battery – Look for residue which may be caught and stuck in the threads of the battery
2. Soak Cotton Swab in Rubbing Alcohol – Pour a tiny amount of the rubbing alcohol into a small container, then soak the head of the cotton swab
3. Clean the Threads: Using a circular motion, gently use the cotton swab to clean the threads of the battery.
4. Allow it to dry: Let the battery dry for a few seconds.

Howard Stern’s opinion on eCigarettes

On the Howard Stern show the famous radio host made his position against eCigarettes (http://www.howardstern.com/news/howards-anti-smoking-and-e-cigarette-views-and-this-day-in-howard-history-ronnie-becomes-a-bodyguard)

On the show and in previous episodes Sal, a writer on the show, has admitted to quit smoking by using electronic cigarettes(https://www.youtube.com/watch?v=SvSWtMW3nzg). On these previous occasions as well Howard and Robin have repeatedly told him to quit cigarettes entirely and not to use eCigarettes. Sal has insisted that quitting cigarettes is a gradual process and that eCigarettes have helped him.

In this particular airing Sal’s doctors claimed that his health has improved due to using eCigarettes (as opposed to regular cigarettes). Howard clearly states that in his opinion eCigarettes have chemicals and hence should be avoided at all costs. In his usual flamboyant style he completely disses the opinion of Sal’s doctors. Though Howard is entertaining as usual his opinion is not entirely accurate due to two reasons:

(a) Sal’s health improved as told by his doctors. This is attributed to the fact that eCigarettes do not have carcinogens or other thousands of other toxic materials found in regular cigarettes. Hence it is a fact that cigarettes are (far) more harmful than eCigarettes

(b) Howard talks about eCigarettes containing chemicals. This is superficially true but these are the same as contained in kids’ candy, ice creams, and many foods. The two ingredients are PG(Propylene Glycol) which is an FDA approved food additive (see https://eciglounge.themagicmist.com/all-posts/what-is-propylene-glycol-2) and Vegetable Glycerin which is naturally found in vegetables. By that measure we should avoid all supermarket foods and vegetables?

In summary non-smokers should not start using eCigarettes but these are definitely a safer alternative to regular cigarettes.
